4. Photo copy Ijazah SMA/SKHU (leges) : 3 Lembar (Leges Sekolah) jika belum ada maka bawa Photo copy Ijazah SMP/Sederajat : 1 Lembar (Leges Sekolah)
5. Pas Photo hitam putih  (3 x 4 = 3 lembar ) dan (4 x 6 = 5 Lembar)

* Ujian metode Computer Based Test (CBT)dan Cek Kesehatan

* Berpakaian rapi dan sopan Serta memakai Sepatu.
